I don't know. Urine in the bladder should have no dirt in it.
Occasionally some dead cells from the bladder wall perhaps, but a
tiny, tiny amount. Being a solution, the urine itself should
not 'settle'. Perhaps we can improve on the intuitions of the
Rishis, as other rishis do?

Or perhaps they speak from experience of which I am unaware. But my
own experience has been that I have noticed no difference at all
between middle and late urine.

The 'dirt' reference was just a metaphor...

I personally believe they percieve something we do
not.
